All-on-X (AOX) Treatment planning and Dental Codes
Here is a general breakdown on how to treatment plan an all-on-5. These are patients who do not want a denture but something permanent in the mouth.
Once patient accepts treatment plan and wants to proceed: Both surgery AND restorative tx plan must be signed by patient.
First appt: Impression for immediate temp denture. This denture must be in-office to do the surgery! So plan the surgery appt accordingly.
The restorative dentist is responsible for the temporary denture. I recommend a thin metal mesh membrane be included into the denture for additional strength.
Phase I: Surgery (which includes extractions, implants, grafts and abutments)
D7210 Extractions- all remaining teeth
D7310 Aveloplasty - Right quadrant AND Left quadrant
D6010 Implants - I usually plan #'s 4, 7, 8, 10, 13
Lower always plan #'s 20, 23, 24, 26, 29
D6104 Bone graft - For every implant site (So use this code 5 times - must match implant #'s)
D4266 Membrane - If needed
D6052 - Abutment - This is a multi-unit abutment. For every implant (So use this code 5 times- must match implant #'s)
The above codes may return high fees, so the office may need to make adjustments.
This total is usually around $13,000-$15,000 per arch.
Phase II: Restorative (Typically 4 months later. This is billed on the day of final impression).
D6114 - Maxillary fixed denture (hybrid)
D6115 - Mandibular fixed denture (hybrid)
This fee is usually around $10,000 each arch but is dependent on the restorative doctor.
Total cost for Tx: $23,000-$25,000 per arch
